Continuing Education Credits
The conference organizers will be applying for continuing education credits for interested participants through the CDC's authority to grant credits. In order to comply with application requirements imposed by the International Association for Continuing Education and Training, you must submit, with your abstract, the following:
- Educational Objective related to the abstract submitted and stated in measurable terms (one is required, but you may submit up to three);
- Biographical Data Form (or a current resume or CV);
- Conflict of Interest Disclosure Form; and
- Speaker Agreement Form. (The Speaker Agreement Form allows the conference organizers to include an abstract and the name of the authors/presenters in official conference publications and to record audio and/or video of the presenter.)
PLEASE NOTE: The electronic abstract submission system will not accept your abstract without the completed forms listed above; these forms are provided electronically on the Web site. You will be allowed to type your name or provide an electronic signature to sign the forms. If an abstract that is submitted with the forms listed above is not selected for a poster or oral presentation, the forms will be destroyed and not used for any other purpose.
